Lando was referred to CSS because he had a swelling under his jaw that wouldn’t go away. Tests revealed systemic cryptococcosis affecting his mandibular lymph node and central nervous system.

Following surgery, antifungal treatment and ongoing care, he is feeling much better. Long-term fluconazole is keeping Lando stable.

Patient history

Early signs

Lando first saw his local vet in May 2023 after a firm swelling appeared under his chin. A fine needle aspirate showed significant pyogranulomatous inflammation and numerous yeast forms consistent with Cryptococcus species.

Even after starting medication, the swelling in his lymph node didn’t go down. In June, he was referred to CSS for further tests and to plan the next step in his treatment.

Diagnosis and treatment

Under the care of Dr Wan Ju Jao, Lando had surgery to remove the affected lymph node and reduce the fungal burden. Testing of the tissue confirmed severe lymphadenitis caused by cryptococcosis, and a serum cryptococcal antigen test showed high levels. Analysis of his cerebrospinal fluid also confirmed the infection had reached his central nervous system.

He received a course of amphotericin B and fluconazole, completing 12 infusions, and continued on oral fluconazole after leaving the hospital.

Outcome

Lando recovered well after surgery and handled his treatment with ease. His appetite and demeanour remained stable, and he has continued to do well on long-term fluconazole. While he remains LCAT-positive, his clinical signs are well-controlled, and he is back to enjoying life at home.
