    In order to provide the best care for your pet please take the time to complete the following questions.

    Dietary requirements

    Unless otherwise instructed, cats will be fed a premium veterinary diet during their stay.

    If cats have any specific dietary requirements (e.g. diabetic, renal or urinary diets or other specific food brands) please note this food will need to be supplied by owners.

    VACCINATION

    All cats must have been vaccinated against Feline Panleukopenia, Feline Rhinotracheitis Virus and Feline Calicivirus (F3) within 12 months prior to admission, unless otherwise agreed upon due to current or ongoing medical conditions. Vaccinations for Feline immunodeficiency Virus or Feline Leukemia virus are not required.

    Parasite control

    All cats must be treated with an approved flea prevention prior to checking in. Any cat found to have external parasites on admission will be treated with a product of CSS choosing and charged to your account. Any cat to be seen passing intestinal worms at any point will be treated with an approved CSS worming treatment and charged to your account. The exceptions to this policy are cats with ongoing medical conditions that prevent a treatment being applied but this needs to be discussed with a feline clinician.

    Refusal at check-in

    Please note that any cat showing symptoms of cat flu (sneezing, eye discharge, etc) may be refused admission to the CSS medical boarding facility. Please phone us prior to your admission date if you have any concerns that your cat is unwell.

    Please let us know if you would like any additional procedures performed during your cats stay. Standard costs will be incurred for these tests and if testing is performed, we would request that our team is able to contact you to discuss results.

    Updates about your pet
    Belongings

    Having something familiar from home such as a t-shirt or small bed can help your cat feel more secure in a different environment and we would recommend bringing something small with them.

    While we take every care to keep personal belongings with each patient, there may be situations that require these to be removed, washed or withheld. This may result in loss of items.

    Although we will try our hardest, please note that CSS does not accept responsibility for loss of collars, bowls, towels, blankets, toys, etc whilst your cay is staying in our care. Tick which of the following items you will be bringing with you:
